Here's what I just posted to comp.text.tex on this: The bugs are supposed to be e-mailed to Frank or one of the other authors. There's a note to that effect in the file. I'd like to note that trying to track ``printing error blue not aligned'' is rather footless. Unless caused by an actual misregistration in the .pdf source file or some mis-imaging of the plates it's going to be different from book to book, depending on how the paper just happened to be handled in-between the print runs --- a number of pages which Frank notes are mis-registered are fine in my copy, while others in copy which aren't noted as such are out-of-register. Even if the .pdf is mis-aligned between colours, some copies will come out okay---such is the analog nature of printing on a press. That said, I _really_ think the duotone screen grabs were rather poorly handled in a number of cases which made them _much_ more vulnerable to registration problems than they ought to've been, and resulting in some pretty ugly results (well at least in my copy, YMMV, well, your registration).
